On the CMS-1500 claim form, Box 19 is a free-text area for extra information about the claim or the patient's condition. This guide explains what the box is for and how Pabau handles it.

Box 19 — Additional claim information
Box 19 is labelled Additional Claim Information (Designated by NUCC). It carries a short narrative when a payer wants context that has no box of its own — anything from a treatment note to a payer-specific instruction. It is one of the most loosely defined areas of the form: what goes here, if anything, depends entirely on the individual payer's adjudication rules.
How Pabau fills it
Pabau leaves Box 19 blank — no free-text claim note is sent or printed. The box is situational, and most payers do not require it for professional claims, so for the typical claim there is nothing to do here.
If a payer does ask for a narrative in Box 19, add it in the Claim.MD portal after sending the claim (open the claim there and use Manage Claim), or hand-fill the box on the printed form before mailing it.
TIP: Supporting documentation does not belong in Box 19 — it travels with the claim as an attachment instead. In the claim's Claim details section, set Attach chart note from case to Yes and Pabau sends the chart note along with the claim as a claim attachment.
