On the CMS-1500 claim form, Box 12 holds the patient's (or an authorized person's) signature and its date. By signing, the patient authorizes the release of any medical or other information necessary to process the claim. This guide explains what the box means and how Pabau fills it.

Box 12 — Patient's signature
Box 12 has two parts: a Signed line and a Date line. It records the patient's release-of-information authorization — permission to share the medical information the payer needs to process the claim. In practice the box rarely carries a wet signature: the accepted convention is the text "Signature on File" (or "SOF"), meaning the practice holds the patient's signed authorization on record. Do not confuse it with Box 13, which is the insured's authorization — often the same person as the patient, but not always.
How Pabau fills it
On the printed claim (Export > Print on the claim), Pabau writes "Signature on File" in Box 12 by default — no setup is needed for this.
When the client has a completed consent form with a drawn signature in Pabau, the full-form print goes one better: it renders the client's actual signature image on the Signed line, together with the date the form was signed. The date is included because NUCC rules require a date to accompany a real signature.
NOTE: The text-only print never renders the signature image — it always keeps the Signature on File wording, so the values stay aligned with the cells of pre-printed red-ink CMS-1500 stock.
On the electronic claim: the release-of-information authorization is asserted automatically when the claim is sent, so there is nothing to set per claim.
TIP: Get consent forms signed at intake. A completed consent form with a drawn signature on the client's record is what puts a real signature — and its date — into Box 12 on the printed claim.
