How to Use the File Upload Component in Forms

In this article, we will walk you through the steps on how to use the file upload component in medical forms.

The File Upload component allows you to collect images or documents from clients, simplifying the process of gathering important files during patient intake. This feature helps ensure that all necessary information is collected efficiently and securely.

Step 1: Navigate to Forms

Log into your Pabau account and go to Setup. From there, click on Forms under the Clinical section.

Step 2: Access a Form

In Forms, you can create a new form or modify an existing one. To edit a form, go to the Edit Form section and open the Components tab. Then, under the "Other" section, click on File Uploads to add or modify the Photo Upload component.

Step 3: Customize the Photo Upload Component

Once you’ve added the Photo Upload component, click on it again to open the settings panel on the right side of the page.

First, you'll need to add a title for the section. This is the heading that will be shown to the client when they fill out the form. For example, you might use a heading like "Photo Upload for Consultation."

Next, below the heading, you’ll find the Photo Number Count field. Here, you can select how many photos you want the client to upload, ranging from 1 to 8. After you enter the number, you’ll need to provide a title for each photo field. For example, if you select "5" for the number of photos, you’ll need to add a title for each photo.

These titles will guide your client in uploading the correct photos. Lastly, you can make the entire Photo Upload section required, meaning clients must upload the photos before submitting the form.

Click Save component to save the component in the form.

Step 4: Save the changes

Once you've added all the information in the photo upload section and saved the component, remember to click Save Form in the top right corner to save the entire form.

How the Section Will Appear to the Client
After the form is saved and sent to the client, this is how the photo upload section will look on their end.

By following these steps, you will successfully set up the Photo Upload component in your form, allowing clients to easily upload the required photos directly through the form.
