On the left-hand side, you’ll find a list of all available field groups used to collect client information.

These include:

  • Phone Number - Landline contact number.

Address

Allows you to collect address-related data, including:

  • House Number/Street

  • City/Town

This is useful for clinics that need physical address information for correspondence or internal records.

General

The General group contains key client-related fields such as:

  • Specific Skin Condition / Goal - Helps track treatment goals.

  • Emergency Contact - A designated contact in case of emergency.

Primary Care Details

Relevant for medical clinics that coordinate care:

  • Primary Care Physician Name

  • Primary Care Clinic Name

These fields ensure your clinic has visibility into a client’s existing medical support network.

Additional General Fields

Includes more optional client-related fields such as:

  • Next of Kin

  • Laser Treatment Preference

  • Assigned Doctor

  • Client Alerts

  • Weight and TestWeight

You can expand or collapse each group using the dropdown arrows. Tick or untick individual fields to control whether they appear on the client creation form.

NOTE: Some fields such as First Name and Last Name are fixed and cannot be moved or removed to ensure that essential data is always captured.

On the right-hand side, you’ll see a live preview of the client creation screen. This preview updates in real-time based on your selections on the left. It shows you how the form will be presented to users when creating a new client card.
