Step 1: Log in to Your Pabau Account
Start by logging into your Pabau account. Once you're in, locate the Setup section at your screen's left bottom corner. Within the Setup menu, look for "Clinical." Under Clinical look for Medical form templates.
Step 2: Create Medical forms
Click on the "Create Form" section and choose between two options: either select from the template store or start with a blank template, just like in the example provided.
Step 3: Choose a Form Type
Choose the appropriate form type from the following options:
Step 4: Create a Consent form
Within the menu located on the left-hand side, you have the option to designate a name for the consent form, link it with the services you want it to apply to, and choose from various components to customize its appearance, such as text fields, checkboxes, or dropdown menu. Once you have completed these steps, click on the "Create" button to finalize the process.
Once done, it will look similar to the one we've created:
Step 5: Booking an Appointment
After booking an appointment with the service you've linked to the consent form, and once patients have completed filling it out, you can locate the form under their client card in the Forms, under the "Charts" section.
