# How to Create Forms Using Capture Forms

## This article explains how you can create forms using Capture Forms in Pabau.

In this article, you will learn how to create a new form in Capture Forms, either from blank or from a starter template, and how to save it.

#### **Step 1: Open Capture Forms**

 Log in to Pabau. In the left sidebar, go to **Marketing → Capture Forms**.

#### **Step 2: Click Create New Form**

Click **Create new form** at the top right of the Dashboard or the **Forms** tab. Click the main part of the button to start from blank, or click the arrow beside it to choose:

- **Start from blank** — opens the builder with an empty form.
- **Use a template** — lets you pick a starting point first.

**Step 3: Choose A Starting Point**

If you chose **Use a template**, the **Choose a starting point** screen opens. Pick one of the starter types:

- **Blank** — Start from an empty form and add your own fields.
- **Lead Capture** — Collect enquiries — name, contact details and a message. (**Feeds a CRM lead**)
- **New/Existing Client Registration** — Onboard new clients or update an existing record. (**Feeds a client record**)
- **Online Consultation** — Pre-appointment medical and treatment questionnaire. (**Feeds a CRM lead**)
- **Lead Capture w/ Photo** — Lead capture plus a photo upload for assessment. (**Feeds a CRM lead**)
- **Image Upload** — A focused form for uploading before & after images. (**Feeds a client record**)

Any templates your clinic has saved are listed under **Your templates**, with the number of forms each has been used in. Select a card (it shows **Selected**), then click **Continue**. Click **Cancel** to go back to your forms.

**TIP:** The badge on each card tells you where submissions go: **Feeds a CRM lead** creates a lead and **Feeds a client record** creates or updates a client. You can change this later in the builder's **Settings** tab.

#### **Step 4: Build Your Form**

The builder opens with the starter's fields already on the canvas. Add, edit and arrange fields, and style the form, until it suits your clinic. Click **Preview** at any time to see the form full screen; click **Close** to return.

#### **Step 5: Save Your Form**

Click **Save Form** under the canvas. A window opens saying **Great! Your form is saved.** and asking **What do you want to do now?**:

- **It's OK.** — I still want to edit this form. (You stay in the builder.)
- **Let's go to Form Settings.** — I need to set up email notifications, confirmation options and more.
- **Share this Form.** — I want to publish and share my form.
- **I finished! Take me back to the Form Manager.** — That's all. (You return to the **Forms** tab.)

Click the arrow beside **Save Form** (**More save options**) for two more choices:

- **Save & View Form** — saves the form and opens a full-screen preview of what you have just saved. Click **Close** to carry on editing.
- **Save as Template** — creates a new template from what is on the canvas and opens the template's settings.

**IMPORTANT:** **Save as Template** does not save the form itself. If you also want to keep the form, click **Save Form** first.
