How to Add SNOMED Fields in Forms

This article explains how you can add the SNOMED fields in the medical forms.

Step 1: Navigate to Setup

Start by logging in to your Pabau account using your login credentials. Then, navigate to the Setup page where you can find "Medical Form Templates" under the Clinical section.
 

Step 2: Locate the Medical Form

On the Medical Form Templates page, locate the form you want to add the SNOMED code to. Click on it to preview and edit it. You can also add the SNOMED code when creating a new form.

NOTE: You can add the SNOMED code to any medical form type except the Intake form.

Step 3: Configure the Medical Form

When editing the medical form, scroll down in the Components tab on your left side. You'll find "Code Lookup" in the Medical section. Drag and drop the component to the form.

Click the pen icon on the far right side of the component to configure the code lookup.

First, enter a title you want to show in the form, then select which code set you want to display on the client card. Click "Save Component" after you've entered the information. 

After you've added the component, ensure to click the "Save Form" button at the top right corner of the page to update the changes. 

Step 4: Navigate to the Client Card

Navigate to the client card by searching for the client's name in the search bar at the top of the page. Type at least three letters for the client's name to appear.

Step 5: Create a Form in the Client Card

Click the "Create" button, represented by a blue plus sign at the bottom right corner of the page, to create the medical form. Choose from any of the form types that you want to add. This action will list all forms that are associated with that type. 

Step 6: Add SNOMED Code

Once you select the form with the code lookup component, locate the field, and enter the code by searching for it in the client card by first adding the hashtag sign (#) and then typing the allergy, problem, social history or any code set you have added to the form. Click "Save Form" to store it in the client card.

After saving the form, the code you added will populate the summary page components in the client card.
