[Medical Forms Components & Fields](https://support.pabau.com/en/pabau2/clinical?hsLang=en#medical-forms-components-fields) # How to Use the Insurance Details and Card Details Components in Forms Two form components collect payment details while a client fills in a form. **Insurance details** asks the client to confirm, update or add their insurance policy, and **Card details** saves a card on file securely. Add them to an intake or registration form and the details land straight on the client's record, with no retyping by your team. **NOTE:** **Insurance details** is available to clinics with Insurance Billing turned on. **Card details** needs online card payments connected to your clinic. You need permission to create or edit forms. **Step 1: Add the Components to Your Form** Go to **Setup** and click **Forms** (under Clinical). Open the form you want to change, or click **Create Form**, choose **Blank Template** and pick a form type such as **Medical History** or **Registration Form**. 1. Open the **Components** tab. Under **Payments**, drag **Card details** into your form. 2. Drag **Insurance details** into your form too, if you want it. 3. Any change is saved to the client's **Insurance** tab straight away, and the form records which policy was confirmed, for example the insurer and membership number. **NOTE:** Insurance can only be recorded once the form is linked to a client. If Insurance Billing is turned off for the clinic, staff see a message instead of the policy, and clients are told the clinic will take their insurance details separately. **Step 3: What the Client Sees in Card Details** The client enters their card in a secure Stripe field and it is saved to their cards on file. Nothing is charged. The form itself keeps only the card type, the last four digits and the expiry date, never the full card number. If a card is already saved, the client can keep it or choose **Use a different card**. **TIP:** A card on file lets you charge insurance shortfalls later without chasing the client.
