Medical forms in Pabau can be customized in various ways to suit different needs. You will see a list of components that can be added to the form. The Basic category is further divided into four subcategories: - General Components - Choice Components - Medical Components - Other Components This guide will explore different form types to demonstrate how these components can be effectively used. ***Using General Components*** - **Text Block** The Text Block component lets you display general information within the form. It’s typically placed at the beginning and can include merge tags to dynamically pull in details like the clinic name, logo, or patient information.  You can also use it for longer text, such as instructions, precautions, or treatment explanations in consent forms. It appears in a larger font to help important sections stand out. To set one: 1. Type your **Question**, for example "Weight". 2. Click **Show advanced settings**. If the question is a measurement Pabau recognises, it fills in the **Suffix** for you and shows "Added automatically from the question. Change or clear it if it's not right." Type a different unit, or clear the box, if it isn't what you want. ![Short answer settings with the question Weight and the Suffix kg filled in automatically](https://support.pabau.com/hubfs/Pabau%202%20Knowledge%20Base/kb/gap-2026-10/250978930897-01-short-answer-suffix.png) *Pabau suggests the unit from the question* Pabau recognises common clinical measurements such as weight, height, waist and other circumferences, temperature, blood pressure, heart rate, oxygen saturation, BMI, blood glucose and cholesterol, and doses such as toxin units or filler volume. The unit matches your clinic's country: for example, weight is suggested in kg in the UK and in lbs in the US. The suggestion only works when the whole question is the measurement ("Weight", "What is your weight?"), not a longer question such as "Weight loss goals", and only for the **Text** and **Number** input types. **NOTE:** Once you type in the **Suffix** box yourself, or open a component that already has a suffix saved, Pabau stops changing it when you edit the question. **NOTE:** For how to use Linked Field, refer to [this article](https://support.pabau.com/en/pabau2/how-to-use-linked-fields-in-medical-forms?hsLang=en). Click the Long Answer on your form, then click **Show advanced settings**. 2. Under **Box size**, choose **Small**, **Medium** or **Large**. New Long Answers start as **Small**. For this example, we’ll refer to the Patient Intake Form, as this component is commonly used in intake forms, consent forms, and other client-facing forms. It functions similarly to Multiple Choice, but clients can select only one option. Click the Drawing component on your form and pick a chart under **Diagram**. Pabau includes a set of built-in charts, so you can start without uploading your own: Female Full Body, Female Face, Female Neck, Male Full Body, Male Neck, Body Front and Back, Male and Female Body, Palm, Back of Hand, Sole of Foot, Toes, Ear, Eye, Eyes and Brows, and Lips. Any charts your clinic has added are listed after them. When a prescription is created in a client’s card, the pre-selected drugs from this form will be automatically included. You can pre-select the lab test categories created under [Services](https://app.pabau.com/setup/services) that can be selected in this field. You can configure the field to search for codes related to problems, family history, diagnoses, medical history, allergies, or social history. Each Code Lookup field can be set to retrieve only one type of code at a time.
