[Medical Forms](https://support.pabau.com/en/pabau2/clinical?hsLang=en#medical-forms) # How to Use Form Analytics to See How a Medical Form Is Used Form analytics shows how one of your medical forms is being used: how often it's sent and completed, who completed it and when, and any set-up problems that could stop it working as it should. Use it to check a form before you rely on it, or to find out whether clients are actually filling it in. **NOTE:** Form analytics is on the medical forms list in Setup, so you need access to Setup to use it. **Step 1: Open Form Analytics** 1. Go to **Setup** and, under **Clinical**, click **Forms**. 2. At the top you'll see when it was created (**Created date**), who created it (**Created By**), its current **Version**, and when it was last completed in the period you're viewing (**Last submission received**). The window opens on the **Last 90 Days**. **Step 3: Review the Summary** 1. On the **Summary** tab, choose a period: **Last 90 Days**, **Last 6 Months** or **Last 12 Months**. 2. Read the three figures for that period: - **Requests** – how many times the form was sent to clients to complete. - **Conversion rate** – completed forms as a share of requests. - **Submissions** – how many times the form was completed. 3. The **Submissions** chart below shows completed forms month by month across all locations. When you edit and save a form, Pabau keeps the older versions, and completions of all of them are counted here. **Step 4: See Who Completed the Form** 1. Click the **Submissions** tab. 2. Each row shows the **Client**, the **Location**, the form **Version** they completed and when (**Submitted on**), newest first, for the period chosen on the **Summary** tab. Click **Warnings**. The list shows every check; ones with a problem show the number of issues, or **Review**, and the rest show a tick. 2. Click a check with issues to see the questions it affects. Hover over the **?** beside a check to see what it means. ![The Warnings list with a check that has issues marked](https://support.pabau.com/hubfs/Pabau%202%20Knowledge%20Base/kb/gap-2026-10/490794933464-04-warnings.png) *Pabau checks the form for common set-up problems* Pabau checks for: - **Fields with incorrect data types** – a question whose label suggests a type (a date, say) but uses a different kind of input. - **Fields which are not linked to the client record** – questions that match a client detail but aren't linked, so answers won't update the client record. - **Form has no linked services** – the form isn't attached to any service, so it won't appear automatically against appointments. - **Form is linked with no default value** – a linked question has no default value, so existing client details won't pre-fill. - **Multiple fields linked to the same client field** – two questions write to the same client detail and will overwrite each other. - **Required fields that cannot be answered** – a heading, text or image is marked as required, so the form can never be submitted. - **Form has no questions** – nothing in the form can be answered. - **Consent form has no signature field** – a consent form should usually capture a signature. - **Multiple default forms for this type** – more than one form of this type is set as the default. - **Linked fields with a mismatched data type** – a linked question's input type doesn't match the client detail it's linked to. For each affected question: 1. Click **Resolve** where it's offered, and Pabau fixes the question for you — for example by linking it to the client record, setting its default value or input type, or removing an impossible "required" setting. The fix is saved to the form straight away. 2. Otherwise, click **Edit** to open the form in the editor and fix it yourself.
