# How to Link an AI Scribe Template to a Treatment Form

## This article explains how you can use AI Templates to save time and boost consistency with AI-generated treatment notes tailored to your unique style.

The **AI Templates** tab of a treatment form links the form to an AI Scribe prompt template. When a clinician then uses AI Scribe in a treatment note on that form, the content AI Scribe writes from the consultation goes straight into the right questions.

**NOTE:** The **AI Templates** tab appears when AI Scribe is enabled for your clinic. It opens once the form has been saved, and only treatment forms can be linked. The tab used to hold free-text AI instructions; prompt templates, which you build in AI Scribe, now do that job.

**Step 1: Create a Prompt Template**

A prompt template sets out the sections AI Scribe writes, such as Subjective, Objective, Assessment and Plan. Templates are created in **Setup → AI Scribe**, under **Prompt Templates**. If your clinic doesn't have one yet, the AI Templates tab tells you: “There are no prompt templates yet. **Step 2: Open the Form's AI Templates Tab**

Go to **Setup**, click **Forms** under Clinical and open a saved treatment form.

1. Click **AI Templates**.
2. Under **Prompt template**, click **Link a prompt template…** and choose a template. You can type to search the list. You'll see **Prompt linked**.

**Step 3: Map the Form's Fields**

1. The badge beside **Prompt template** shows where you are: **Not linked**, then **Fields not mapped** once a template is chosen, then **AI ready** when the form's questions are mapped.
2. Click **Map fields**.

**Step 4: Link Each Question to the Template**

The mapping panel lists your form's questions on the left and the template's sections and subsections on the right.

1. Under **Form questions**, click a question.
2. Under **Prompt subsections**, click each subsection whose content should fill that question. A tick shows it's linked, and content from several subsections is added together. Click a ticked subsection again, or **Unlink all**, to undo.
3. The counter at the top shows how many questions are linked, for example **4 / 4 fields linked**. Any question left unlinked won't be filled in by AI Scribe, and the panel warns you about it.
4. Click **Save & close**.

**Step 5: Check the Form Is AI Ready**

1. When the mapping is saved, the badge reads **AI ready**. The form is ready to use with AI Scribe.

The link stays in place when you edit and save the form later. To change templates, choose another one from the list; to remove the link, click the **×** beside the template. **Manage templates** opens AI Scribe setup in a new tab, where you can also see every AI-ready form.
