# How to Record a Client Problem (New Client Card)

## This article covers how to record client problems in the client card.

Recording a client's problems and diagnoses keeps their clinical picture accurate and up to date. Each one can carry an ICD or SNOMED code, be linked to the appointments it relates to, and be marked active or inactive.

**Step 1: Navigate to the Client Card**

Click the search bar at the top of the page and type at least three letters of the client's name. Pabau filters the results as you type. Click the client's name to open their card.

**Step 2: Open the Diagnoses Section**

1. In the client card, click **EMR** in the navigation at the top (this tab is labelled **Chart** for US accounts).
2. Click the **Diagnoses** card to see the client's problem list.

**NOTE:** If you don't see the **Diagnoses** card, the Diagnostics feature isn't turned on for your business, or the card has been hidden with **Customize** on the EMR overview.

**Step 3: Add a Problem**

1. Click **Add Problem**. It sits at the top right of the section, or in the middle when the client has no problems recorded yet.

The **Enter Problem** window opens. If the client has an appointment today, Pabau links the record to it automatically and shows **Appointment · auto-linked** at the top of the window. Fill in the details:

1. **Record as**: choose **Problem** or **Diagnosis**. Problems appear on the client's problem list. A diagnosis belongs to its appointment; tick **Add to problem list** if it should also show on the problem list.
2. **Problem**: start typing to search by name or code. Results come from your clinic's diagnostic code set (such as ICD-10, ICD-11 or SNOMED) and your clinic's own Medical Conditions list. Select the right entry.

The other fields are optional:

- **Show details**: add or adjust the ICD Version, ICD Code and SNOMED CT Code.
- **Related appointment(s)**: link more of the client's appointments to this record.
- **Diagnosis Date**: the date the condition was diagnosed. If you leave it blank, today's date is used when you save.
- **Notes**: anything relevant about the diagnosis.
- **Diagnosis Status**: **Active** or **Inactive**. Active problems appear in the Active tab and count towards the client's conditions total.

**Step 4: Reuse a Past Diagnosis**

When the client already has problems or diagnoses on record, you don't need to search for the code again. The **Past diagnoses** panel on the right of the window lists them. It appears when you add a new record, not when you edit one.

1. **Past diagnoses** shows "Previously recorded problems and diagnoses, newest first". Each entry shows its date, whether it is Active or Inactive, its code and the clinician of any appointment it is linked to.
2. Click **Use diagnosis** to copy that entry's name and codes into the new record. Then set the date, notes and status for this occasion.
3. Click **Hide history** to close the panel, or **Past diagnoses** to open it again.

**TIP:** To record several problems in a row, click **Save & Add Another** after each one instead of **Save**. The window stays open for the next one.

**Step 5: Save the Problem**

Click **Save**. The problem appears in the Diagnoses list, in the tab that matches its status, and is counted in the conditions total on the Medical History banner at the top of the EMR overview.
