[New Client Card](https://support.pabau.com/en/pabau2/new-client-card?hsLang=en) # How to Link a Treatment Note or Form to an Insurance Case (New Client Card) Insurers often want the clinical note behind a visit. In Pabau you can link a treatment note or form to the client's insurance case, either while you write it or later from the case. Pabau keeps a PDF copy of each linked form with the case, so the documentation is in one place when you bill. **NOTE:** Linking forms to cases is available once Insurance Billing is switched on under **Setup → Business Details → Features**, and the client needs an open insurance case. See [How to Manage Insurance Case Details (New Client Card)](https://support.pabau.com/en/pabau2/how-to-manage-insurance-case-details?hsLang=en). **Step 1: Link the Note While You Write It** Open a new treatment note for the client as usual. Then: 1. The **Select Case** window lists the client's open insurance cases, grouped by insurer, with each case's authorisation code. Click the case this visit belongs to. The header now shows the insurer and the case name. If the client has none, the window says **No open insurance cases for this client.** The link is made when the note saves, not before. Pabau shows **Attaching to case…**, then **Linked to case** with the form's name. The note and the appointment it's for are both linked to the case, and Pabau saves a PDF copy of the note with it. **NOTE:** If you see **Form PDF could not be generated**, the note is linked without a PDF copy and won't be sent with insurance claims until you attach it again from the case (Step 2). If you see **Couldn't link to the case**, your note is still saved; link it from the case instead. **Step 2: Add Forms From the Insurance Case** Use this for forms that are already completed, or to add several forms to a case. On the client card, click **Insurance**, then **View** on the case. In the **Notes** section you'll see how many **Attachments** and **Forms** the case has, with the names of the linked forms. 1. Click the **Forms** tab. The **Documents** tab next to it is for files saved in the client's documents. 2. Type in **Search forms on file…** to find a completed form, then click it to add it. Added forms show as a tag above the list and read **Added**; click the **×** on a tag to take a form off. 3. If the client has no completed forms yet, the list reads **No completed forms on file.** You can do the same when you create a case: click **+ Add Case**, expand **Attachments** and open the **Forms** tab. **TIP:** For US clinics billing through Claim.MD: when you send a claim with a chart note but don't pick a document, Pabau sends the newest PDF on the visit's insurance case, and a form linked to the case counts.
