[Medical Forms Components & Fields](https://support.pabau.com/en/pabau2/clinical?hsLang=en#medical-forms-components-fields) # How to Let Clients List Their Medications in a Form The **Medications** form component lets clients list the medicines they take, with the dose and how often, inside a form such as your medical history questionnaire. When the form is completed, each medication is added to the client’s Medications on the client card, so your team doesn’t have to type it in again. **NOTE:** Medications is different from the **Drugs** component, which prescribes medicines from your drug list. Medications can’t be added to prescription or lab forms. **Step 1: Add the Medications Component to a Form** 1. Go to **Setup**, click **Forms** under Clinical and open the form, for example your medical history questionnaire. 2. On the **Components** tab, find **Medications** in the **Medical** group and drag it onto the form. You can also click it to add it to the end of the form. 3. Click the component on the form to open its settings. The **Question** is set to **Current medications**; change it if you like, then click **Save component**. 4. They enter the **Medication name** (required) and, if they know them, the **Dosage**, **Frequency**, **Start date** and any **Notes**. **Currently taking** is switched on; they switch it off for a medicine they no longer take. 2. They click **Add**. Click **+ Add medication** in the form. 2. In the **Add Medication** window, enter the **Drug Name** and, if you have them, the **Dosage**, **Frequency**, **Start Date** and a **Note**. Leave **Active** switched on for a medicine the client still takes. 3. Click **Save**. On the client card, open **EMR**, choose **Prescriptions** and click the **Medications** tab. They also appear in the Medications section of the Facts Sheet when you write a treatment note. **NOTE:** A medication whose name is already on the client’s list isn’t added a second time.
