How to Create a Treatment Form

In this article, we'll guide you through the process of creating a treatment form.

Step 1: Navigate to Medical Forms

Start by logging into your Pabau account. Once you're in, locate the Setup section at your screen's left bottom corner. Within the Setup menu, click on Medical Form templates under Clinical.

Step 2: Create a Medical Form

Click on the "Create Form" section and choose between two options: either select from the template store or start with a blank template.

Choose the appropriate form type from the following options:

Step 4: Customize the Treatment Form

In the General menu on the left-hand side, you can assign a name to the treatment note.

Navigate to the Components menu on the right to customize the treatment form's appearance using various components, such as text fields, multiple or single-choice questions, and dropdown menus.

Since this form is for treatments, it should include a diagram with enabled plotting and drawing capabilities. To add a diagram, locate the "Diagram" option in the "Other" section of the Components menu.

Upon selecting "Diagram," the Component settings will appear on the right side, allowing you to specify a name or question relevant to the diagram.

If you wish to be able to draw and plot on the diagram, make sure to toggle the "Enable drawing" and "Enable plotting" options. 

Click on "Save component" to add the diagram to the form.

If you wish to reorder the components, click on a component and drag and drop it to the desired position.

The treatment forms created for a client can be found in the "Chart" or "EMR" section of their client card, under "Treatment notes".
