How to Create a Medical History Questionnaire
This article will guide you through the process of creating a medical history questionnaire/patient intake form.
Step 1: Navigate to Medical Forms
Start by logging into your Pabau account. Once you're in, locate the Setup section at your screen's left bottom corner. Within the Setup menu, click on Medical form templates under Clinical.
Step 2: Create a Medical Form
Click on the "Create Form" button and choose between two options: either select from the template store or start with a blank template.
Step 3: Choose a Form
Choose the appropriate form type from the following options:
Step 4: Customize the Form
In the General menu on the left-hand side, you can assign a name to the form.
Navigate to the Components menu to customize the form's appearance using various components, such as text fields, multiple or single-choice questions, and dropdown menus, depending on your needs.
After a client fills out the form, you can locate the form in the "Chart" or "EMR" section of their client card, under "Forms".
