NOTE: These components are available to users with an active Care PLUS plan. To learn more about upgrading to Care Plus, refer to this guide.
Step 1: Navigate to Forms
Start by logging into your Pabau account using your login credentials. Then, navigate to the Setup page by clicking the "Setup" button located at the bottom corner of the left sidebar menu. Find the "Forms" tab under the "Clinical" section.
Step 2: Create/Edit Medical Form Template
On this page, you'll find all your medical form templates listed. If you haven't created any templates yet, navigate to the "Create Form" button on the top right corner of the screen and proceed to create a new medical form template.
To add components to an existing medical form template, click on the template to open it on the screen, then make your changes as needed.
Step 3: Drag and Drop Components
The left side of the page lists all the available components. Scroll to locate the "Checkbox grid" and "Single-choice grid" components.
To add a component to the medical form template, either click on it to have it appear on the screen or simply drag and drop it onto the page.
Step 4: Configure Components
Once you've added the components to the template, click the pen icon to edit and proceed to enter the questions and answers.
- Single Choice Grid Question
In a single-choice grid, respondents can select only one option per row. - Multiple Choice Grid Question
In a multiple-choice grid, respondents can select more than one option per row.
Once you've added the questions and answers in the component settings, the changes will appear on the edit screen with a grid view. To save the updates, click the "Save component" button in the bottom right corner of the component settings.
Step 5: Save Changes
To add the new component so clients can answer this question in the future, click the “Save Form” button in the top-right corner.
By following these steps, you can easily include the single-choice and multiple-choice grid checkboxes in your medical form templates.
