# How to Complete a Medical Form Before Your Appointment

## In this guide, we’ll walk you through how to complete and submit forms through the clinic’s Client Portal.

**NOTE:** Your clinic decides which options you see, so some of the steps below may look slightly different or be skipped.

Your clinic may ask you to fill in a form before your appointment — for example, a medical questionnaire or a consent form. Doing it in advance saves time on the day and gives your practitioner the information they need. In this article, you will learn the two ways to complete a form: from the link your clinic sends you, or from your client portal.

**Option 1: Use the Link in Your Email or Text Message**

When a form is needed, your clinic sends you an email or text message with a link to it. Open the link, answer the questions and click **Save and continue** at the end of each form. You don’t need to sign in. When you have finished every form, you will see **Thank you** — your answers have been sent to the clinic.

**NOTE:** Form links expire after a while. If you see **This link has expired**, contact your clinic for a new link, or complete the form from your client portal as shown below.

**Option 2: Complete the Form in Your Client Portal**

**Step 1: Open the Appointment**

1. Sign in to your client portal and click **Appointments** in the menu. On a phone, tap the menu button (☰) at the top left first.
2. Click the appointment the form is for.

**Step 2: Start the Form**

If the appointment has a form waiting, you will be asked straight away: **\[Clinic name\] would like you to complete a form**.

1. Click **Complete** to start now, or **Maybe later** to come back to it.

**Step 3: Or Start It From the Forms Section**

If you clicked **Maybe later**, you can start the form at any time from the appointment’s **Forms** section:

1. Forms you still need to fill in are marked **To complete**.
2. Click **Complete** under the form’s name.

**Step 4: Fill In and Save the Form**

1. Answer the questions. Questions you must answer are marked, and the form tells you if you have missed one. Where a signature is needed, sign in the box with your finger or mouse — click **Clear** to start again.
2. Click **Save and continue** at the bottom of the form.

You will see **Thank you. Your answers have been sent to the clinic.** and return to your appointment, where the form now shows as **Completed**. If there are more forms, complete each one the same way until you see **You’ve completed every form for this appointment.**

**TIP:** Booking online? If a treatment needs a form, you may be asked to fill it in as part of the booking, before you pay.

**NOTE:** Some forms are completed by your practitioner with you at the appointment. If a form says **Your clinician will complete this with you at the appointment**, there is nothing for you to do in advance.
