On the CMS-1500 claim form, Box 7 holds the address of the insured — the person who holds the insurance policy, named in Box 4. This guide explains what the box means and how Pabau fills it.

Box 7 — Insured's address
Box 7 carries the policy holder's street address, city, state, and ZIP code, with a cell for a telephone number. When the patient is their own policy holder — the most common case — the address here is the same as the patient's address in Box 5.
How Pabau fills it
The claim editor has no separate insured-address fields. Pabau always mirrors the patient's address (Box 5, from the Patient section of the claim) into Box 7 — on the printed form and on the electronic claim alike. Whatever address the claim carries for the patient is what the payer sees for the insured, so keeping the patient's address correct covers both boxes at once.
CAUTION: If the policy holder genuinely lives at a different address than the patient, that address cannot currently be entered in Pabau. Add it in the Claim.MD portal after sending (open the claim under Manage Claim and edit it there), or hand-fill the box on the printed form.
Most payers accept the mirrored address when the patient is the subscriber, which is the common case — for self-insured patients there is nothing to correct.
On the electronic claim: the patient's address is sent as the insured's address as well, so the claim never leaves Box 7 empty.
