On the CMS-1500 claim form, Box 4 names the person who holds the insurance policy — the insured. That may be the patient, or someone else such as a spouse or parent. This guide explains what the box means and where Pabau takes the value from.

Box 4 — Insured's name
Box 4 carries the full name of the policy holder, written last name first. It is required on every claim, paper or electronic. When the patient is their own policy holder, Box 4 simply repeats the patient's name from Box 2; when the coverage belongs to someone else, Box 4 is where that person is identified.
How Pabau fills it
The claim's Insurance section (CMS-1500 boxes 1a, 4, 7, 11) has two fields for this box: Insured first name (if not the patient) and Insured last name.
When the patient holds the policy, leave both fields blank — Pabau fills the insured name from the patient automatically, so Box 4 always matches the patient's name exactly, with no retyping. When someone else holds the policy, enter that person's first and last name in these fields before validating the claim.
On the printed claim (Export > Print on the claim), the name renders in the form's standard order: last name, then first name.
On the electronic claim: the same rule applies — blank fields are replaced by the patient's name before the claim is sent, so the wire always carries a complete insured name.
TIP: Only fill these fields when the policy holder is genuinely a different person. Leaving them blank for self-insured patients keeps Box 2 and Box 4 identical, which is exactly what payers expect.
