On the CMS-1500 claim form, Box 2 identifies the patient — the person who received the care the claim bills for. This guide explains what the box holds and where Pabau takes the value from.

Box 2 — Patient's name
Box 2 holds the patient's full name, written in the order Last Name, First Name, Middle Initial. The last and first name are required on every claim, paper or electronic; the middle initial is optional.
How Pabau fills it
The name lives in the Patient section of the claim editor, which is labeled with the CMS-1500 boxes it covers (2–5). When an invoice issued to the insurer becomes a claim, First name and Last name prefill from the client record, and you can correct them on the claim before validating or sending. Any Patient field you leave blank falls back to the client record at validation.
The claim editor collects the first and last name only — there is no middle-name field. On the printed form (Export > Print), the name renders as Last, First, and a middle initial is never printed. A middle name, when present in the claim data, is included on the electronic claim.
On the electronic claim: the patient's name travels in Loop 2010CA of the 837P file — or in Loop 2010BA when the patient is also the policy holder (the subscriber).
TIP: Because the Patient section prefills from the client record, the cleanest place to fix a misspelled name is the client card itself — every claim you create afterwards starts out correct. A correction typed on the claim is the quick fix for the claim in front of you.
